Advanced Directives:
An Advanced Directive is a document that directly affects your healthcare if you are not able to do so. It is a patient right to have access to complete an Advanced Directive at any time. It is regulated by Federal, State, HCFA, and Joint Commission requirements.
At MMC, the option to complete an Advanced Directive is give to the patient during their admissions process. A green sticker on their file signifies whether or not they wish to meet with someone to assist them in completing their Advanced Directive. A Social Worker from Patient Management will come and complete form with patient if he/she so desires. For any questions, please call 521-2220.
